How do I access services?

Access to our services at Charak Center for Health & Wellness is simple. All you have to do is call 1-855-4CHARAK (or 1-855-424-2725) and a member of our intake department will be ready to assist you. You may also see some of our service under the Services tab 

What information do I need to set up an appointment?

The easiset way to set up an appoinment is by clicking on the "Book an Appointment" button on the top center part of the page. A member of our staff will be notified about your appontment request and will get back to you with a confirmation.

If you prefer to set up an appoinment by phone, there will be a few pieces of information you will need to provide a member of our intake staff to complete the process. You will need to provide the following:

• Your name
• address
• phone number 
• age
• social security number
• the type of service you are requesting 
• your insurance card

Once you provide this information, our intake staff will take care of the rest and will provide you with a scheduled appointment. The process is simple and should only take about 10 minutes of your time.

What will happen during my first appointment? How long will my appointment last?

For your first appointment, you will need to arrive 30 minutes before your scheduled appointment time to fill out necessary paperwork. It is also highly recommended that you bring any bottles along with you of medications that you are currently being prescribed.

Our clinical staff is very caring and will take care of your needs. Sure, you will be asked a lot of questions about yourself, but, just remember the purpose of this is to gain the information to be able to help you best. Just remember, there are no “right” or “wrong” answers, just be open and honest and you will notice that the process will be very smooth.

Typically, your first appointment may be one of the longest as you may spend anywhere between 30 to 60 minutes with your clinician. Factoring the extra half hour to account for paperwork for your appointment, you may need to a lot for at least 60 to 90 minutes for your initial visit.

What insurances do you accept?

At Charak Center for Health and Wellness, we accept Medicaid, Medicare, Bureau of Workers Compensation and all major private insurances. Please refer to this link for a complete list .
